Following your application to join Runshaw College you provided us with some information about possible support needs and we are now asking you to provide us with more details. Please complete and return this form. So that we can support you fully, please give us as much detailed information as possible. This form may take around 15 minutes to complete and you may want to ask your parents/carers for help in doing it.

After receiving your form, we may be in contact to discuss any support that can be offered and may draft a Student Support Plan (SSP) for you which will identify any difficulties that you face along with strategies to aid your learning.  Once you enrol, this Support Plan will be visible on your Student Portal and will be made available to your teachers.
